There are a handful of questions, most optional, but the free form comment section is probably the most important to me. ANYTHING that pops into your head is helpful, especially vague impressions and gut reactions if you can find words for them. I'm particularly stumped when people ask me what kind of music I write. I usually don't know what words to use to describe it.

I intend to use this feedback for two purposes. The first is to create even better music in the future. The second is to help me describe, market, and focus my musical abilities more effectively.
